病因

1.维生素D:
维生素D在调节哮喘的广泛免疫过程和抗炎反应方面发挥着至关重要的作用。来自几种过敏性哮喘动物模型的实验室证据表明,维生素D可能在逆转哮喘肺部的气道重塑或气道炎症中发挥作用1,2。证据还表明,维生素D可以预防哮喘恶化3。对哮喘患者的研究发现,血液中维生素D水平低或缺乏与哮喘的几个指标有关4-6。
观察性研究表明,与产前维生素D摄入量较低的妇女相比,维生素D摄入量较高的孕妇的孩子患喘息和哮喘的风险较低7-9。此外,一项针对轻度至中度持续性哮喘儿童的纵向研究表明,在4年的时间里,维生素D水平低与严重哮喘恶化的风险较高相关10。另一项研究发现,与维生素D水平较高的儿童相比,6岁时维生素D水平较低的儿童在14岁时更容易患哮喘11。
为了确定因果关系,美国国立卫生研究院(clinicaltrials.gov)正在进行干预研究,以评估维生素D预防或降低哮喘风险的能力。两项随机对照临床试验正在进行中,以确定母亲补充维生素D是否可以预防儿童哮喘(NCT00920621;NCT00856947)。一项针对青少年和成人哮喘患者的临床试验将测试维生素D的补充是否会影响首次上呼吸道感染或严重恶化的时间(NCT00978315)。另一项针对成年人的临床试验将测试在低剂量对照药物中添加维生素D以预防哮喘症状和发作的效果(NCT01248065)。
2.维生素E:
许多研究表明,摄入维生素C、E、黄酮类化合物和硒等抗氧化剂可以减少哮喘引起的支气管收缩。维生素E是四种生育酚和四种生育三烯酚的统称,它们具有抗氧化和抗炎特性。研究表明,维生素E可防止炎性细胞因子的释放,并特异性抑制IL-4的基因表达12。

3.维生素C:

一项随机对照试验证明了抗氧化剂在哮喘中的作用。患有持续性哮喘的儿童,如果补充了ω-3脂肪酸、维生素C或锌,肺功能会得到改善。当儿童接受所有三种营养素时,他们的肺功能改善的程度甚至比接受单独营养素时更大20。另一项针对八名哮喘受试者的临床试验发现,与服用安慰剂的受试者相比,每天服用1500 mg维生素C两周的受试人哮喘症状评分显著改善21。
4. 欧米伽3脂肪酸:

5.益生菌:

6.硒:

8.镁:

9.姜黄素:

10.番茄红素:

11.黄酮类化合物:
类黄酮是多酚(存在于水果、蔬菜、红酒和茶中),具有抗氧化和抗炎特性。黄酮类化合物与改善肺功能有关49。以下黄酮类化合物/黄酮类化合物植物已在哮喘的背景下进行了研究:
- 槲皮素:槲皮素的部分化学结构与色甘酸相似,色甘酸是一种肥大细胞稳定剂,有时用于治疗哮喘50。在一项研究中,膳食中摄入大量槲皮素(存在于葡萄酒、茶和洋葱中)、柚皮素(存在于橙子和葡萄柚中)和橙皮素(发现于橙子和柠檬中)与哮喘患病率较低有关51。几种哮喘动物模型已经证明了槲皮素的抗炎特性。在一项研究中,单剂量口服槲皮素在培养和体内都会引起显著的支气管扩张52。在另一项研究中,口服槲皮素显著降低了炎症细胞因子IL-5和IL-4的水平,并抑制了肺部粘液的产生53。在另一种动物模型中,当槲皮素在哮喘诱导物质之前给药时,它显著抑制了所有哮喘反应54。
- 原花青素:原花青素是碧萝芷的主要成分,它从法国海松树皮中提取。原花青素是一种强大的抗氧化剂,可以中和自由基55。一项随机安慰剂对照试验发现,与安慰剂组相比,轻度至中度哮喘儿童,除了每日和/或抢救性吸入器外,还服用碧萝芷®4周,其肺功能和哮喘症状显著改善。此外,与对照组相比,治疗组能够更频繁地减少或停止使用救援药物56。在最近的一项试验中,在患有稳定、可控哮喘的成年人中发现了类似的结果,与仅吸入皮质类固醇或安慰剂相比,他们使用碧萝芷®作为辅助药物57。
- 银杏叶:银杏叶似乎是一种有效的哮喘治疗方法58-60。在一项研究中,银杏叶提取物被添加到皮质类固醇中2周。研究人员发现,与纯药物组或安慰剂组相比,银杏叶治疗患者的痰液中炎症细胞明显较少,这表明银杏提取物可以缓解与哮喘相关的气道炎症60。在哮喘动物模型中,过敏激发后用银杏进行治疗,提取物抑制了肺组织中嗜酸性粒细胞和气道中粘液分泌细胞的释放61。
12.款冬(蜂斗菜):
蜂斗菜自古以来就用于治疗各种疾病。从该草药中分离出的四种活性成分—蜂斗菜素(Petasin)、异蜂斗菜素(Isopetasin)、S-蜂斗菜素(S-petasin)和S-异蜂斗菜素(S-isopetasin)。可以抑制白三烯(与哮喘相关的炎症介质)62。
一些研究小组已经检验了蜂斗菜对哮喘的有效性,并取得了令人鼓舞的结果。在一项针对64名成年人和16名儿童和青少年的开放标签试验中,哮喘患者接受了为期两个月的蜂斗菜提取物治疗,然后是可选的两个月治疗期。数据显示,在整个研究过程中,所有测量到的症状都有所改善,40%的患者能够减少传统哮喘药物的摄入63。另一项研究发现,蜂斗菜疗法与吸入皮质类固醇相结合,可以减轻哮喘症状64。
实验动物模型的结果显示了S-蜂斗菜素作为哮喘治疗剂的潜力。在过敏原激发的哮喘动物的皮肤下施用S-蜂斗菜素,可以显著减缓炎症细胞和介质的产生,并放松支气管,这表明S-蜂斗菜素具有抗炎和支气管扩张的特性65。一个测试蜂斗菜提取物的动物模型对哮喘小鼠观察到了类似的抗炎作用66。
13.乳香:
有证据表明,乳香树胶树脂中的化合物调节导致哮喘症状的炎症过程。乳香提取物已被证明可抑制白三烯的合成并抑制IgE和炎性细胞因子的释放67-69。此外,一项使用哮喘动物模型的研究发现,使用乳香治疗可以减轻哮喘症状,并防止肠道微生物组中与哮喘相关的细菌多样性减少70。
在一项对照试验中,32名哮喘患者被分配接受标准药物治疗,外加每天500mg的乳香提取物,该提取物被配制成植物组分以增强吸收,或单独接受标准药物处理。四周后,乳香组对急性吸入器治疗的需求减少,但标准护理组没有减少71。在另一项试验中,40名哮喘受试者被随机分为两组,一组接受300mg乳香提取物或安慰剂治疗,每天三次,为期六周。虽然接受安慰剂治疗的受试者中只有27%的人有改善,但70%接受乳香提取物治疗的人在肺功能测试中表现改善,呼吸困难、喘息和发作次数等症状减少。乳香组还表现出嗜酸性粒细胞计数减少和红细胞沉降率(ESR)降低,这分别是过敏和炎症的测量指标72。
含有乳香的草药组合也显示出治疗哮喘的前景。在一项针对36名轻度至中度哮喘受试者的随机安慰剂对照试验中,与安慰剂相比,每天200mg的乳香提取物(标准化为含有15%乙酰基-11-酮-β-乳香酸,AKBA)加上木橘(Aegle marmelos)提取物(标准化为含有0.3%欧前胡素,Imperatorin)的一对一组合在14天后减轻了哮喘症状。到第56天,在接受草药组合治疗的患者中,IL-4水平下降,干扰素γ水平升高,这表明免疫学从过敏性气道炎症转移。此外,接受草药组合治疗的受试者的呼气峰流速(呼吸功能的衡量标准)增加了90.00升/分钟,而接受安慰剂治疗的受测者的呼气峰值流速为43.85升/分钟73。另一项包括63名哮喘患者的试验发现,与安慰剂相比,每天服用三次、持续四周的乳香、姜黄素和甘草的组合显著降低了白三烯C4(LTC4)的水平和氧化应激水平74。
乳香还表现出抗病毒和抗菌活性,因此可能有助于预防或治疗哮喘患者的肺部感染75,76。
14.穿心莲:
穿心莲内酯是从穿心莲中提取的一种化合物,穿心莲是一种传统上用于治疗呼吸道感染的亚洲植物77。在哮喘的临床前研究中,已经发现穿心莲内酯可以抑制参与哮喘的T细胞活性,清除自由基,调节炎症信号通路,减少炎症细胞因子的释放,减少免疫细胞对肺组织的浸润,并改善轻度至重度哮喘78-80。其他临床前研究表明,穿心莲内酯对气管平滑肌有放松作用,因此可能有助于减少气道收缩81。
15.藏红花:
藏红花是一种烹饪香料和药用草本植物。负责藏红花颜色的类胡萝卜素色素,番红花素(Crocin)和番红花酸(Crocetin)也具有抗炎、减少氧化应激、抗退行性、抗抑郁等生物活性82。临床前研究表明,从藏红花中提取的番红花素可以调节炎症信号,降低免疫系统的过敏样高反应性83。在哮喘动物模型中,番红花醛(Safranal,另一种番红花活性成分)降低IgE水平,抑制肥大细胞活化,并平衡T细胞因子以缓解哮喘;它还抑制了参与过敏反应的分子,如组胺和LTC484。藏红花可以通过对气道平滑肌的松弛作用进一步减少气道收缩85。
在一项包括80名轻度至中度哮喘患者的随机安慰剂对照试验中,连续八周每天100mg藏红花改善了肺功能测试的表现,降低了哮喘中升高的抗热休克蛋白抗体70(抗HSP70)以及炎症标志物高敏C反应蛋白(hs-CRP)的水平86。同一试验的另一份报告表明,藏红花可以减少哮喘症状和吸入器的使用频率。此外,藏红花还能降低血压、甘油三酯和LDL胆固醇水平,以及嗜酸性粒细胞和嗜碱性粒细胞的数量87。
16.娃儿藤:
印度娃儿藤(Tylophora indica)叶子已被研究为治疗哮喘症状的潜在药物。在60年代末和70年代初发表的研究中,印度娃儿藤比对照组更有效地缓解了哮喘症状88,89。不幸的是,没有新的研究严格评估印度娃儿藤作为哮喘的治疗方法。然而,研究人员最近汇集了旧试验的数据,发现在对变量进行调整后,治疗效果仍然显著90。作者得出的结论是“……印度秋刀兰显示出改善肺功能的潜力……”。
